摘要
目的探讨肺炎支原体(MP)感染与冠心病(CHD)发生的相关性及MP引起冠心病的致病机制,为临床诊治提供参考。方法将医院2011年1月-2012年1月收治的CHD患者45例作为观察组,另选取同期健康体检者45人为对照组,检测并比较两组患者MP抗体及总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)和高密度脂蛋白(HDL-C)等指标水平。结果观察组MP-IgG和MP-IgM阳性率分别为73.33%和62.22%,而CRP升高率为71.11%,对照组分别为37.78%、28.89%和24.44%,观察组明显高于对照组(P<0.05);冠心病组中MP-IgG阴性组患者TC、TG和LDL-C水平分别为(4.02±0.38)mmol/L、(1.21±0.22)mmol/L和(2.32±0.21)mmol/L,MP-IgG阳性组分别(5.89±0.45)mmol/L、(2.04±0.28)mmol/L、(3.43±0.27)mmol/L,MPIgG阴性组均明显低于MP-IgG阳性组,而MP-IgG阴性组患者HDL-C水平为(1.34±0.15)mmol/L,MP-IgG阳性组为(0.94±0.13)mmol/L,MP-IgG阴性组显著高于MP-IgG阳性组,两组比较差异有统计学意义(P<0.05)。结论 MP感染与冠心病的发生相关,可能与MP影响脂质代谢或通过炎症反应参与冠心病的病理过程有关。
OBJECTIVE To explore the relationship between Mycoplasma pneumoniae infection and coronary heart disease and discuss the pathogenesis of coronary heart diseases induced by M. pneumoniae. METHODS Totally 45 patients with coronary heart disease who were treated in the hospital from Jan 2011 to Jan 2012 were assigned as the observation group, while 45 healthy people who underwent physical examination were chosen as the control group, then the levels of the indicators including the M. pneumoniae antibodies, total cholesterol (TC),triacylg- lycerol (TG), low-density lipoprotein (LDL), and high-density lipoprotein (HDL) were determined and compared between the two groups. RESULTS The positive rate of MP-IgG was 73.33% in the observation group, significantly higher than 37.78% in the control group;the positive rate of MP-IgM was 62.22% in the observation group, significantly higher than 28.89 %in the control group; the elevation rate of CRP was 71.11% in the observation group, significantly higher than 24.44% in the control group (P〈0.05). Of the patients with coronary heart diseases, the levels of TC, TG, and LDL of the patients with negative MP-IgG were respectively (4.02 ±0.38)mmol/L, (1.21±0. 22)mmol/L, and (2. 32±0. 21)mmol/L, significantly lower than (5. 89±0.45) mmol/L, (2. 044±0.28)mmol/L, and (3.43±0.27)mmol/L of the patients with positive MP-IgG; however, the level of HDL-C of the patients with negative MP-IgG was (1.34±0.15)mmol/L, significantly higher than (0.94 0.13)mmol/L of the patients with positive MP-IgG(P〈0.05). CONCLUSION The M. pneumoniae infection is associated with the incidence of coronary heart disease, which may be related to the impact of M, pneumoniae on lipid metabolism or the inflammation reactions' involving in pathogenesis of coronary heart disease.
