摘要 各种原因所致的慢性肾损伤最终均会发展为肾纤维化。肾纤维化( renal fibrosis)是肾脏慢性病理重塑及瘢痕形成的过程,在这个过程中,在炎症、损伤、细胞因子等多种因素的作用下,肾脏固有细胞增殖、转分化,大量肌成纤维细胞( myofibro-blast,MyoF)形成,合成和分泌大量胶原蛋白、纤连蛋白等细胞外基质( extracelluar matix,ECM),同时,ECM的降解被抑制,导致大量的ECM积聚,正常的组织结构最终被ECM等纤维化物质所代替。由此可见,MyoF 在肾纤维化中起关键作用[1~5]。目前有研究发现,人附睾上皮分泌蛋白( human epididymis pro-tein-4,HE4)是肾纤维化时MyoF中上调最多的基因,并且认为HE4有可能是肾纤维化MyoF形成的标志物并参与ECM的积聚[1]。本文就 HE4及其参与肾纤维化的研究进展作一综述,以期为肾纤维化早期诊断的新生物学标志物及其治疗的新靶点提供参考。
