Pharmaceutical care of diuretic treatment in patients with heart failure

目的:探讨临床药师参与心力衰竭患者临床药物治疗的实践方式。方法:临床药师在参与心衰患者药物治疗过程中以利尿治疗为切入点,在患者发生利尿治疗效果不佳时提出治疗方案的调整建议,并对患者进行用药监护和用药教育。结果:临床药师根据患者的疾病状态、饮食和并用药物等因素,提出改善利尿效果的合理化建议,成功缓解患者心衰症状。结论:临床药师通过开展利尿治疗的药学监护参与到心力衰竭的治疗之中,有利于保障心衰患者获得安全、有效的药物治疗。

OBJECTIVE To explore the ways of clinical pharmacists participating in individualized medication and pharmaceutlcal care tor patients with heart failure. METHODS Selecting pharmaceutical care of diuretic treatment as a new starting point in the care of patients with heart failure, clinical pharmacists provided substitution medications for worse diuretic response, pharmaceutical care and medication education of patients. RESULTS According to morbid state, diet and combination drugs, clinical pharmacists proposed rational suggestions to improve the diuretic response, and finally got satisfactory therapeutic efficacy. CONCLUSION The participation of clinical pharmacists in diuretic treatment in heart failure patients makes the drug therapy safer and more rational.
