中西医结合治疗上消化道穿孔40例

Treatment for 40 Cases of Upper Gastrointestinal Perforation by Integrative Medicine

目的:观察在内镜下金属钛夹治疗的基础上以健脾汤联合三联西药综合治疗上消化道穿孔的临床疗效。方法:将80例患者随机分为观察组、对照组各40例,2组均给予常规内镜下手术与三联西药综合治疗,观察组同时加用健脾汤治疗。结果:总有效率治疗组为95.0%,对照组为82.5%,2组比较差异显著(P<0.05)。腹痛、烧心、反酸等症状积分治疗后2组均明显降低(P<0.05),观察组降低更明显(P<0.05)。术后肠蠕动恢复时间、术后住院时间观察组短于对照组(P<0.05)。并发症发生率观察组为5.00%,对照组为17.50%。2组比较差异显著(P<0.05)。结论:在内镜下金属钛夹治疗的基础上以健脾汤联合三联西药综合治疗上消化道穿孔临床疗效显著,并发症发生率低。

Objective：To explore clinical effects of spleen-invigorating decoction and triple therapy in treating upper gastrointestinal perforation on the foundation of endoscopic hemoclip. Methods： Eighty patients were randomized into the observation group and the control group, both groups were given with endoscopic surgery and triple therapy, the observation group with spleen-invigorating decoction. Results： Total effective rate of the observation group was 95.0%, superior to 82.5%of the control group with significant difference （P〈0.05）. The scores of the symptoms such as abdominal pain, heartburn, sour regurgitations and others were decreased in both groups significantly （P〈0.05）, the decrease of the observation group was more notable （P〈0.05）. The observation group was shorter than the control group in recovery time of intestinal peristalsis after the surgery and hospitalization time after the surgery （P〈0.05）. Complication incidence of the observation group was 5.00%, less than 17.50%of the control group with notable difference （P〈0.05）. Conclusion：Based on the treatment of endoscopic hemoclip, spleen-invigorating decoction combined with triple therapy are effective in treating upper gastrointestinal perforation with lower incidence of the complications.
