摘要 早产儿视网膜病变(retinopathy of prematurity,ROP)是一种发生于早产、低体质量儿的视网膜血管增生性病变,具体机制尚不明确,目前认为早产、低出生体质量、吸氧、感染和营养不良等诸多因素可能是ROP发生的诱因。近年的研究发现,与生长发育及营养状况相关的胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)和胰岛素样生长因子结合蛋白-3(insulin-like growth factor binding protein-3,IGFBP-3)与ROP的发生相关。基于新生儿出生后体质量和血浆IGF-1水平变化预测ROP的筛查模式,即WINROP已在多个国家进行了多中心临床试验,可以有效提高ROP的筛查效率,提高新生儿血浆IGF-1/IGFBP-3浓度水平,有望在ROP预防方面发挥作用。 Retinopathy of prematurity (ROP) is a retinal vasoproliferative disease mainly happened in preterm birth and low birth weight infants. The etiological factors of ROP are still unclear,low gestational age ,low birth weight, oxygen supplementation,infection and malnutrition appears to be correlated with ROP. In recent years, a series studies have identified insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) are important relevant factors, either. The weight IGF-1 Neonatal ROP (WINROP) algorithm, which is based on serial measurements of neonatal body weight and serum IGF-1 to predict proliferative ROP has been invented, and a mul- ticenter large sample clinical trials about WINROP algorithm has been conducted in many countries, which demonstrates WINROP is efficient in screening ROP. Furthermore,improving serum IGF-1/IGFBP-3 concentration may play an important role in the prevention of ROP.
