双钢板治疗复杂性胫骨平台骨折22例 被引量:2

Curative Effect of Double-plate in Complex Tibial Plateau Fracture

摘要 目的:探讨双钢板固定治疗复杂性胫骨平台骨折患者的临床疗效。方法:选取2010年1月-2013年1月在我院行双钢板内固定术治疗复杂性胫骨平台骨折病例22例,其中:SchatzkerV型14例,SchatzkerVI型8例。对临床治疗结果进行回顾性分析。结果:全部病例术后均获得随访,随访时间24个月,均达骨性愈合。1例出现手术区域皮肤的浅表感染,在经过局部冲洗、换药以及应用足量的抗生素治疗后恢复;1例出现骨筋膜室综合征,及时行切开减压处理后伤口愈合;1例发生创伤性骨性关节炎,后期行膝关节松解术后恢复。参照HSS膝关节功能评定量表对膝关节功能做出评定:优10例、良7例、可3例、差2例;优良率为77.3%。结论:采用双钢板治疗复杂性胫骨平台骨折具有固定牢固可靠、便于早期进行功能锻炼,术后并发症少等优势,是治疗此类骨折较为理想的方法,可以获得良好临床疗效。 Objective:To evaluate the clinical efficacy of double-plate fixation for complex tibial plateau fracture. Methods: From January 2012 to January 2013, 22 patients being treated with double-plate in our hospital were analyzed retrospective- ly, including 14 cases of Schatzker V and 8 cases of Schatzker VI. Results:All patients were followed up for an average time of 24 months and all had bony union. Postoperative wound infection appeared in 1 case and was healed after domestic douche, changing medicine and a amount of antibiotic. Osteofascial compartment syndrome appeared in 1 case and the wound was completely healed for timely decompression. Knee stiffness appeared in one case and was recovered after arth- rolysis. According to HSS knee joint function score system, the result were excellent in 10 cases, good in 7,fair in 3 and poor in 2. The excellent and good rate was 77.3%. Conclusion: Double-plate fixation for complex tibial plateau fracture is a satisfactory method for it's rigid fixation,early exercise and less complication.
