硝酸甘油联合卡托普利治疗慢性肺源性心脏病心力衰竭的效果观察

Effect observation of nitroglycerin combined with captopril treating chronic pulmonary heart disease complicated with heart failure

目的：探讨硝酸甘油联合卡托普利治疗慢性肺源性心脏病心力衰竭的临床效果。方法选取本院2012年8月～2014年5月收治的62例慢性肺源性心脏病心力衰竭患者为研究对象，将其随机分为两组，各31例。对照组给予吸氧、抗感染、强心等常规治疗，观察组在对照组基础上给予硝酸甘油、卡托普利治疗。比较两组的治疗效果、治疗前后血流变化及血气分析指标。结果观察组中显效16例（51.61%），好转13例（41.94%），总有效率为93.55%，对照组中显效11例（35.48%），好转11例（35.48%），总有效率为70.97%，观察组的总有效率明显高于对照组（P〈0.05）。观察组治疗后的血流及血气相关指标改变明显优于对照组（P〈0.05）。结论在吸氧、抗感染等常规治疗的基础上给予慢性肺心病心力衰竭患者硝酸甘油联合卡托普利治疗能明显缓解患者的临床症状，改善血流变化及血气情况，促进患者恢复，值得临床进一步应用。

Objective To explore the clinical effect of nitroglycerin combined with captopril treating chronic pulmonary heart disease complicated with heart failure. Methods Sixty-two patients with chronic pulmonary heart disease complicated with heart failure admitted into our hospital from August 2012 to May 2014 were selected as research objects and they were evenly divided into two groups in random.In the control group,conventional treatment like oxygen inhalation, anti-infection, and cardiotonic treatment and so on were provided,while in observation group, on the basis of the control group,nitroglycerin and captopril were added.The therapeutic effect,blood flow change,and indexes of blood gas analysis before and after treatment were compared in the two groups. Results In the observation group,there was significant effectiveness in 16 cases （51.61%） and improvement in 13 cases （41.94%）,the total effective rate was 93.55%.In the control group,the number of cases in significant effectiveness and improvement was 11 （35.48%） and 11 （35.48%） in turn,the total effective rate was 70.97%,the total effective rate in the observation group was obviously higher than that in the control group （P〈0.05）. Besides,changes of the related indexes of blood flow and blood gas analysis in the observation group after treatment were remarkably superior to those in the control group （P〈0.05）. Conclusion On the basis of conventional treatment including oxygen inhalation and anti-infection and so on,nitroglycerin combined with captopril is provided for patients with chronic pulmonary heart disease with cardiac failure,which can obviously alleviate their clinical manifesta-tions,improve blood flow and blood gas condition,and promote recovery.It is worthy of further application in clinic.
