摘要
目的 研究微创颅内血肿清除术对高血压脑出血患者神经功能恢复状况的影响。方法 选择2011年1月~2013年6月本院收治的109例高血压脑出血患者,随机分为试验组和对照组。试验组56例患者采用微创颅内血肿清除术治疗,对照组53例患者采用保守方法治疗。持续治疗数周后,对两组患者的神经功能恢复状况及生活能力进行比较分析。结果 试验组与对照组治疗前斯堪的纳维亚卒中量表(SSS)评分、Barthel指数(BI)评分和改良Rankin量表(MRS)评分差异无统计学意义(P〉0.05);治疗后21 d,试验组SSS评分和BI评分均显著高于对照组(P〈0.05);治疗后90 d,试验组MRS评分显著低于对照组(P〈0.05)。治疗后试验组日常生活能力(ADL)量表评级Ⅰ级所占比例为37.74%,显著高于对照组的18.75%,差异有统计学意义(χ2=4.4358,P=0.035)。结论 微创颅内血肿清除术对高血压脑出血治疗效果良好,患者神经功能恢复状况好,值得临床推广应用。
Objective To study the influence of minimally invasive intracranial hematoma on nerve function in patients with hypertensive intracerebral hemorrhage. Methods 109 cases of patients with hypertensive intracerebral hemorrhage treated in our hospital from January 2011 to June 2013 were selected and randomly divided into experimental group (56 cases) and control group (53 cases).Minimally invasive intracranial hematoma was taken to the experimental group,con- servative treatment was taken to the control group.After continuous treatment for several weeks,neurological recovery and living conditions in both groups were compared. Results Before treatment,the Scandinavian stroke scale (SSS) score,Barthel iondex (BI) score and modified Rankin scale (MRS) score of the two groups had no significant difference (P〉0.05);21 days after treatment,the SSS score and BI scores of experimental group were significantly higher than those of the control group (P〈0.05);90 days after treatment,the MRS score of experimental group was significantly lower than that of the control group (P〈0.05).After treatment,the activities of daily living (ADL) scale of rating level Ⅰ of the experimental group was 37.74%,higher than 18.75% of the control group,there was significant difference (χ2=4.4358,P= 0.035). Conclusion Minimally invasive intracranial hematoma makes good effect on hypertensive intracerebral hemorrhage,neurological functional recovery is good,is worthy of clinical application.
