不同输血方法在自身免疫性溶血性贫血患者中的效果分析

Effect analysis of different transfusion methods in patients with autoimmune hemolytic anemia

目的:探讨洗涤红细胞输注、悬浮红细胞输注、洗涤红细胞输注联合血液置换在自身免疫性溶血性贫血中的治疗效果。方法:选取本院2009年12月至2013年2月收治的自身免疫性溶血性贫血患者132例,根据输血方法分为3组,洗涤红细胞输注患者44例为A组,悬浮红细胞输注患者44例为B组,洗涤红细胞输注联合血液置换患者44例为C组。比较3组患者输血前后的临床指标,分析3种输血方法的临床疗效和并发症情况。结果:输血后,3组患者红细胞计数、血红蛋白均明显上升,网织红细胞、胆红素均明显下降。C组患者红细胞计数、血红蛋白、治疗总有效率均明显高于A组和B组,网织红细胞、胆红素均明显低于A组和B组,差异均有统计学意义(P<0.05)。B组患者红细胞计数、血红蛋白、治疗总有效率、并发症发生率均高于A组,网织红细胞、胆红素均低于A组,C组患者并发症发生率高于A组和B组,但差异均无统计学意义(P>0.05)。结论:洗涤红细胞输注联合血液置换是治疗自身免疫性溶血性贫血的有效方法,可显著改善临床指标,提高临床疗效。

Objectives： To investigate treatment effect of washed red blood cell transfusion,suspended red blood cell transfusion,washed red blood cells transfusion with blood replacement in autoimmune hemolytic anemia.Methods： 132 patients with autoimmune hemolytic anemia were selected in hospital from December 2009 to February 2013. According to methods of transfusion,patients were divided into three groups. 44 patients treated washed red blood cell transfusion as A group. 44 patients treated suspended red blood cell transfusion as B group.44 patients treated washed red blood cells transfusion with blood replacement as C group. Clinical indexes were compared between three groups before and after blood transfusion. Clinical efficacy and complications were analyzed. Results： After blood transfusion,red blood cell count and hemoglobin increased obviously in three groups,while reticulocyte and bilirubin decreased significantly. Red blood cell count,hemoglobin,total effective rate in C group were significantly higher than those in A group and B group. Reticulocyte and bilirubin in C group were significantly lower than those in A group and B group. Differences were statistically significant（ P 〈 0. 05）.Red blood cell count,hemoglobin,total effective rate and complication rate in B group were higher than in A group. Reticulocyte and bilirubin in B group were lower than in A group. Complication rate in C group was higher than those in A group and B group. Differences were not statistically significant（P 〉 0. 05）. Conclusion： Washed red blood cells transfusion with blood replacement is an effective method in treatment of autoimmune hemolytic anemia,which can significantly improve clinical indexes and clinical efficacy.
