131例胎儿肾积水的产前诊断及转归 被引量:1

摘要 目的:应用超声检查研究胎儿肾积水的声像图特点,同时了解胎儿肾积水的演变规律及生后治疗情况,为产前干预及产后处理提供依据。方法选择在本院就诊的孕妇,产前超声检查诊断出胎儿肾积水共131例152只肾(有胎儿为双肾积水),出生后随访24个月,分析152只肾不同诊断时间、122例可复性和不可复肾盂积水不同肾盂前后径(APD)预后及不同APD的106只可复性肾盂积水的消退时间。结果盂积水宫内消失、可复性和不可复盂积水以发生比例分别为19.7%、69.8%和10.5%;孕周<33周诊断肾盂积水者宫内消失比例较高,不可复性比例较低,与≥33周比较,差异有统计学意义;APD值增大,肾积水不可复的比例增加,消退的时间越长,产前消退的比例越低,差异有统计学意义。结论产前肾积水大部分都是生理性的,可自发消退;生后仍存在肾积水的患儿,应定期随访观察。APD值≥15 mm组往往存在病理性梗阻,应高度重视。
