原发性弥漫结节型肝癌与多发结节型转移性肝癌的CT表现

Primary Diffuse Nodular Liver Cancer and Multiple Nodules Type CT Manifestations of Metastatic Liver Cancer

目的:探讨和分析原发性弥漫结节型肝癌与多发结节型转移性肝癌患者的CT表现及诊断价值,并总结相关经验。方法:选取2011年1月-2013年12月在笔者所在医院确诊且接受治疗的原发性弥漫结节性肝癌患者26例和18例多发结节型转移性肝癌患者为研究对象,对上述研究对象的临床资料和CT扫描结果进行整理和分析,总结诊断经验。结果:整理发现,原发性弥漫结节型肝癌患者CT表现主要特点如下:平扫或增强扫描时,肝脏各叶体积比例均明显异常,不成比例;平扫时,肝脏各叶呈现等密度或弥漫性低密度小结节,且边界模糊;增强扫描时,发现肝脏各部位病灶未出现明显强化或呈环状强化;对于多发结节型转移性肝癌患者,其CT表现主要有以下特点:平扫时,肝脏各叶呈多发混杂密度结节影,且边界不清晰;增强扫描后,病灶边缘均出现明显的环状强化,但最外层显示为低密度,并无明显强化,"牛眼征"和"门静脉瘤栓"患者依次为14例和2例。结论:原发性弥漫结节型肝癌患者和多发结节型转移性肝癌患者CT表现均具备较为明显的临床特征和特异性,通过联系患者的临床资料及其他检测结果,有利于病症的鉴别和确诊。

Objective：Discussion and analysis of primary diffuse nodule with multiple nodules of liver cancer patients with metastatic liver cancer CT manifestations and diagnostic value,and summarizes the experience.Method：Selection from January 2011 to December 2013 in our hospital diagnosis and treatment of 26 patients with primary diffuse nodular liver cancer and 18 cases of multiple nodules in patients with metastatic liver cancer as the research object, the clinical data of the study object and CT scans for collection and analysis,summarized the experience in diagnosis.Result：Finishing found that primary diffuse nodules in patients with cancer of the liver CT performance main characteristics as follows：scan or enhanced scanning,liver lobe volume ratio were significantly abnormal,disproportionate.Scan,each lobe liver isodensity or diffuse low density nodules,and the fuzzy boundaries.Enhanced scan,each part of the liver lesion does not appear obviously improved or show cricoid improved.For multiple nodules in patients with metastatic liver cancer,the CT performance mainly has the following characteristics：scan,all of right hepatic lobe,multiple mixed density nodules,and the boundary was not clear.Edge enhancement scan,lesions were apparent annular enhancement,but the outermost displayed as low density,and no obvious reinforcement,＂bull’s-eye＂and＂hua＂portal vein tumor patients in 14 cases and 2 cases in turn.Conclusion：Primary diffuse nodules in patients with liver cancer and multiple nodules in patients with metastatic liver cancer CT has more obvious clinical features and specificity,by contacting the patient’s clinical data and test results,to identify and diagnose disease.
