Clinical practice of clinical pharmacists in the optimized pharmaceutical care process

目的:以优化药学服务流程为切入点,探讨临床药师开展药学服务的内容与方法。方法:阐述临床药师优化药学服务流程的目的、形式、内容、重点、效果,并进行典型案例分析。结果:通过实施优化药学服务流程,使病人、医护人员及临床药师均受益。结论:优化药学服务流程可成为临床药师融入临床开展药学服务的切入点,体现临床药师的价值。

Objective： To explore contents and methods of pharmaceutical care by clinical pharmacists with optimized phar- maceutical care process as an entry point. Methods.. The purposes, forms, contents, focuses and efficiency of optimized phar- maceutical care process were described, and then typical cases were analyzed in detail. Results： Through the implementation of optimized pharmaceutical care process, patients, medical staffs and clinical pharmacists all benefitted quite substantially from it. Conclusion： Optimized pharmaceutical care process could be used by clinical pharmacists as an entry point to carry out phar- maceutical care, and is also a reflection of the value of clinical pharmacists.
