品管圈对血液透析患者动静脉内瘘穿刺点渗血的护理效果

The effect of quality control circle on the incidence of fistula puncture site bleeding on hemodialysis patients

目的探讨品管圈对血液透析患者动静脉内瘘穿刺点渗血的护理效果。方法选择中国中医科学院望京医院血液净化中心2013年8月~2014年1月进行规律维持性血液透析的患者,将120例符合研究入组标准的血液透析患者采用随机对照方法分为观察组和对照组,各60例,观察组（品管圈护理组）男33例,女27例,平均年龄（61±10）岁,采用品管圈护理方法;对照组（传统护理组）男35例,女25例,平均年龄（68±10）岁,采用传统护理方法。开展期为6个月。探讨实施对策对动静脉内瘘穿刺点渗血的影响。结果运用品管圈后传统护理组渗血发生率为23.3‰,品管圈护理组渗血发生率为12.4‰,两组渗血发生率经χ2检验比较,差异有统计学意义（P〈0.05）。结论运用品管圈活动能够降低血液透析患者内瘘穿刺点渗血发生率,有助于提高护士工作的主动性和解决护理问题的能力。

Objective To explore the nursing effect of quality control circle （QQC） to hemodialysis patients whose arte-riovenous fistula puncture site were bleeding. Methods 120 cases of maintenance hemodialysis patients were included in Blood Purification Center of Wangjing Hospital of China Academy of Traditional Chinese Medicine. The treatment time was from August 2013 to January 2014. They were divided into observation group and control group. There were 60 cases of patients in observation group as much as in control group. There were 33 cases of male patients and 27 cases of female patients in observation group. Their average age was （61±10） years old. The QQC nursing was adopted in observation group. There were 35 cases of male patients and 25 cases of female patients in control group. Their av-erage age was （68±10） years old. The traditional methods of care was adopted in control group. The observation time was 6 months. The effect of QQC on arteriovenous fistula puncture site bleeding was explored. Results The bleeding rate in QCC care group was 12.4‰, the bleeding rate in traditional care group was 23.3‰, the difference was statistically significant （P〈0.05）. Conclusion The use of QCC can reduce the bleeding incidence of hemodialysis patients arteriove-nous fistula puncture site. It will help to initiative the work of nurses and improve the alility of solving nursing problem.
