精神病院与综合医院医疗纠纷原因的比较

Comparison of medical disputes causes between mental hospital and general hospital

目的探讨精神病院与综合医院医疗纠纷原因的异同,以便于因地制宜地制订有效的预防措施。方法采用回顾性对照研究,对海南省安宁医院及海口市人民医院2007年1月~2013年12月间发生的医疗纠纷进行分析。结果海南省安宁医院的医疗纠纷中门诊投诉比例为15.15%,同期海口市人民医院的医疗纠纷中门诊投诉比例为59.06%,二者比较差异有统计学意义（χ2=49.04,P〈0.05）。投诉人员性质比较：海南省安宁医院以患者家属投诉为主,海口市人民医院以患者本人投诉为主,二者比较差异有统计学意义（χ2=498.42,P〈0.05）。常见医疗纠纷原因：海南省安宁医院以工作责任心不强为主,海口市人民医院以服务态度与沟通欠佳为主,二者比较差异有统计学意义（χ2=41.63,P〈0.05）。结论精神病院与综合医院的医疗纠纷原因存在差异,这可能与二者的就医环境和患者的疾病特点相关。

Objective To explore the similarities and differences of medical disputes causes between mental hospital and general hospital, in order to establish effective preventive measures. Methods With a retrospective case-control study, the medical disputes were compared between Hainan Provincial Anning Hospital and People＆#39;s Hospital of Haik-ou City from January 2007 to December 2013. Results 15.15% medical disputes occurred in Outpatient of Hainan Provincial Anning Hospital, and that was 59.06% in People＆#39;s Hospital of Haikou City, the difference was significant （χ2=49.04,P〈 0.05）. Comparison of the complaints： the family members of patients were major in Hainan Provincial Anning Hospital, meanwhile, patients were major in People＆#39;s Hospital of Haikou City, the difference was significant （χ2=498.42, P〈0.05）. The comparison about causes of medical disputes：lack of work responsibility was major cause in Hainan Provincial Anning Hospital, meanwhile, service attitude and poor communication was major cause in People＆#39;s Hospital of Haikou City, the difference was significant （χ2=41.63, P〈 0.05）. Conclusion There is difference between mental hospital and general hospital in reason of medical dispute, which may be associated with medical environment and disease feature.
