经椎弓根植骨治疗胸腰椎爆裂性骨折的临床价值

Clinical value of pedicle bone grafting in the treatment of thoracolumbar burst fracture

目的：评价经椎弓根植骨治疗胸腰椎爆裂性骨折的临床效果。方法选取2010年1月~2012年9月本院收治的胸腰椎爆裂性骨折患者65例，随机分为两组，其中观察组33例，经损伤椎体后路钉棒固定后，行椎弓根植骨，对照组32例，仅经损伤椎体后路钉棒固定，观察并比较两组术后3个月的椎体高度压缩率、损伤节段椎体Cobb角、骨折愈合情况及内固定并发症发生率。结果观察组术后3个月的椎体高度压缩率、损伤节段椎体Cobb角与对照组比较，差异有统计学意义（P〈0.05）；观察组骨折愈合26例（78.8%），骨折不愈合7例（21.2%），对照组骨折愈合17例（53.1%）例，骨折不愈合15例（46.9%），两组的骨折愈合率比较差异有统计学意义（P〈0.05）。两组的内固定并发症发生率比较差异无统计学意义（P〉0.05）。结论经椎弓根植骨治疗胸腰椎爆裂性骨折的临床效果好，骨折愈合速度快，值得临床推广应用。

Objective To evaluate the clinical efficacy of pedicle bone grafting in the treatment of thoracolumbar burst fracture. Methods 65 cases of patients with thoracolumbar burst fracture in our hospital from January 2010 to Septem-ber 2012 were selected and randomly divided into two groups.After posterior pedicle screw fixation of vertebral injury, pedicle bone grafting was used in observation group （33 cases）,posterior pedicle screw fixation of vertebral damage was used in control group（32 cases）.The compression rate of vertebral height,the damage vertebral segment Cobb angle,frac-ture healing and incidence rate of internal fixation complication in two groups after surgery of three months was ob-served and compared respectively. Results After surgery of three months,vertebral height compression rate,injured seg-ments vertebral Cobb angle in observation group compared with control group respectively,with statistical difference（P〈0.05）.There were 26 cases （78.8%）of fracture healing and 7 cases （21.2%）of bone nonunion in observation group,there were 17 cases （53.1%）of fracture healing and 15 cases （46.9%）of bone nonunion in control group.The fracture healing rate between two groups was compared,with statistical difference（P〈0.05）.Incidence rate of internal fixation complication in two groups was compared,with no statistical difference （P〉0.05）. Conclusion The pedicle bone grafting in the treat-ment of thoracolumbar burst fracture has good clinical efficacy and fast speed of fracture healing,is worthy of clinical promotion and application.
