左炔诺孕酮宫内缓释系统治疗子宫腺肌病的临床效果观察

Clinical effect observation of levonorgestrel releasing intrauterine system in the treatment of adenomyosis

目的：探讨左炔诺孕酮宫内缓释系统对子宫腺肌病的临床治疗效果。方法以本院2010年1月~2012年12月收治的患有子宫腺肌病的78例患者为研究对象，均接受左炔诺孕酮宫内缓释系统治疗，观察其临床治疗效果。并对患者治疗前后的痛经程度、血雌二醇的变化以及月经量进行统计学分析比较。结果在放置左炔诺孕酮宫内缓释系统12个月后，疼痛消失者有54例，疼痛明显减轻有17例，无显著效果的5例，2例患者发生脱落。放置3、6、12个月后，月经量及疼痛程度与放置前比较，差异有统计学意义（P〈0.05），血雌二醇的变化差异无统计学意义（P〉0.05）。结论左炔诺孕酮宫内缓释系统对子宫腺肌病的治疗具有明显的临床效果，同时卵巢的功能也没有受到显著的抑制作用，值得在临床上进一步推广应用。

Objective To explore the clinical therapeutic effect of levonorgestrel releasing intrauterine system （LNG-IUS） in the treatment of adenomyosis. Methods Seventy-eight patients with adenomyosis admitted into our hospital from January 2010 to December 2012 were selected as research objects.The LNG-IUS was provided in all patients,and the clinical therapeutic effect was observed.The degree of dysmenorrhea,change of estradiol in blood,and menstrual blood volume before and after treatment were statistical analyzed and compared. Results Twelve months after placing LNG-IUS,summarization of therapeutic effect indicated that pain greatly alleviated in 17 cases and disappeared in 54 ones.Except that,there was no obvious effect in 5 cases and falling off in two patients.After 3,6,12 months,the menstrual blood volume and comprehensive score of pain had significant difference compared with before therapy （P〈0.05）.There was no significant change in blood estradiol before and after treatment （P〉0.05）. Conclusion LNG-IUS can obtain a remarkable clinical effect on treating adenomyosis and no significant inhibition on ovarian function occurred,which is worthy of further application and expansion in clinic.
