摘要
目的:观察并评价胸腔镜辅助肺叶切除术治疗Ⅰ/Ⅱ期周围型肺癌的临床效果。方法选择2012年5月~2013年3月于本院手术治疗的Ⅰ/Ⅱ期周围型肺癌患者76例,根据治疗术式的不同,分为对照组(n=34)和观察组(n=42),对照组采取传统开胸肺叶切除术治疗,观察组采取胸腔镜辅助肺叶切除术治疗,比较两组的手术情况、术后并发症及预后情况。结果两组的手术时间、术中出血量、术后引流量、术后镇痛药使用时间、住院时间及并发症发生率比较,差异有统计学意义(P〈0.05)。两组术后12个月局部复发率比较差异无统计学意义(P〉0.05)。两组上肢活动受限率比较差异有统计学意义(P〈0.05)。结论胸腔镜辅助肺叶切除术是治疗Ⅰ/Ⅱ期周围型非小细胞肺癌的有效术式,具有创伤小、患者术后恢复快的特点,且不增加患者术后近期复发率,利于患者术后生活质量的提高,远期手术效果有待进一步观察。
Objective To observe and evaluate the clinical effect of video-assisted mini-thoracotomy in the treatment of stage Ⅰ/II peripheral lung cancer. Methods 76 patients with stage Ⅰ/II peripheral lung cancer from May 2012 to March 2013 were selected and divided into control group (n=34)and observation group (n=42)according to the different surgical treatment,the control group received traditional pulmonary lobectomy,while the observation group received video-assisted mini-thoracotomy,the intercomparsion of the result of surgery situation,postoperative complication and prognosis situation between two groups was compared respectively. Results Operation time,intraoperative blood loss, postoperative flow,postoperative analgesic use time,length of hospital stay and incidence rate of complication in two groups was compared respectively,with statistical difference(P〈0.05).Local recurrence rate of postoperative 12 months in two groups was compared,with no statistical difference(P〉0.05).Restricted activity rate in two groups was compared,with statistical difference(P〈0.05). Conclusion Video-assisted mini-thoracotomy is an effective surgical method in the treat-ment of stageⅠ/II peripheral non-small cell lung cancer.Compared with traditional pulmonary lobectomy,there are the characteristics of smaller operation wound and faster postoperative recovery in video-assisted mini-thoracotomy,and there is not the recent recurrence rate increasing in patients and it is helpful to improve the quality of patients’life,but further observation is needed to determine its long term effectiveness.
