Analysis and intervention of irrational medical advice in the pharmacy intravenous admixture service of our hospital

目的：统计分析本院静脉用药集中调配中心不合理医嘱，为临床合理用药提供参考。方法对本院静脉用药集中调配中心2012年6月~2013年10月的全部静脉用药医嘱进行统计分析。结果共审查出357条不合理医嘱，不合理医嘱类型主要有溶媒选择不当、载体量不适宜、配伍禁忌、用法用量不适宜等。结论本院静脉用药集中调配中心用药医嘱存在一定的不合理现象，药师应积极发挥对不合理医嘱的干预作用，以促进临床合理用药。

Objective To analyze statistically the irrational advice in the pharmacy intravenous admixture service （PI-VAS）of surgical pharmacy in our hospital and provide reference for clinical rational drug use. Methods The medical advice of PIVAS in the surgical pharmacy in our hospital was summarized and amalyzed statistically from June 2012 to October 2013. Results A total of 357 irrational medical advices were reviewed.The type of the irrational medical advice contained incorrect selection of solvent,improper frequency and carrier amount,excessive liquid adding amount,improper dosage,incompatibility and so on. Conclusion Based on the problem of irrational medical advice in PIVAS,pharmacist should play an important role in intervening the irrational medical advice for promoting the rational drug use.
