纠正概念大力发展我国基础医疗卫生服务事业--从“初级卫生保健”中文误译说起

Rectifying Misconception and Emancipating for Development of Robust Primary Health Care in China- A Showcase in Correction of Misunderstanding of Primary Health Care in Chinese Translation “初级卫生保健”

英文中的基础医疗卫生服务(Primary Health Care,PHC)被中文错误地理解和翻译成"初级卫生保健"已有半个多世纪。这个错误在我国政府和医药卫生领域产生了一系列误导,例如在国家医药卫生发展的整体方向和布局上,在政府医疗卫生政策的制定和人才培养上,在国家医药卫生财政拨款的方向上都产生了负面影响。本文从英文语言单词本身的分析,从医药卫生学术领域的理论和内涵,从世界各国医疗卫生服务的实践经验,以及对我国医药卫生事业产生的不利影响等多个角度,阐述中文"初级卫生保健"对英文Primary Health Care的误译和对这个概念本身的完全误解。在我国进入新一轮医药卫生改革,发展我国基础医疗卫生服务事业,建立基层医生制度的今天,希望引起我国医药卫生学术界和我国政府的卫生行政部门重视,尽快纠正这一概念误译,正本清源,以促进我国新一轮医药卫生改革健康发展。

English ＂ Primary Health Care （PHC） ＂ has been misunderstood and wrongly translated into Chinese as ＂ Entry Level Health Care, 初级卫生保健＂ for more than a half century in China. This clear mistake has misled the government and health care field with many serious consequences, such as in overall health care ethics and development strategy, in govern- ment- policy making for infrastructure planning and health care work force education, and in government＇s health care financing. This article elucidates the mistake in translation of PHC into Chinese ＂ Entry level health Care, 初级卫生保健＂ and the clear misunderstanding of the PHC concept, from various angles： the basic English meaning of ＂ Primary＂ , the concept of PHC in health care field, the PHC experience in the world and the harmful consequences in China. In China- current ongoing new re- form of health care with core development in PHC and new general practice system, it is hoped that scholars in the health care field, health care professionals, and officers in the government fully understand the PHC concept and rectify harmful effects in various sectors, and promote the advance of the new health care reform in right direction.
