临床药师对1例急性脑梗死患者的药学监护

Pharmaceutical Care for one Case of Acute Cerebral Infarction by Clinical Pharmacist

目的探讨脑梗死患者的药学监护,协助医生用药,确保患者用药安全。方法结合脑梗死治疗原则,协助主治医生制定治疗方案,对1例急性脑梗死患者制定个体化的药学监护计划并实施全程药学监护。结果和结论对脑梗死患者实施药学监护,及时发现患者治疗中存在的问题,避免严重后果的出现,提高了药物治疗的安全性、有效性。

Objective To discuss the mode and the pharmaceutical care for patients with cerebral infarction and improve rational drug use.Methods An individualized pharmaceutical care plan was designed for one case of cerebral infarction patient by considering therapeutic principles of the acute cerebral infarction and analyzing treatment regimen.Results and Conclusion By providing pharmaceutical care for patients with cerebral infarction,the problems related to treatment are able to be identified in time,and the serious consequence is avoided,safety and effectiveness of treatment are ensured.
