切开复位内固定治疗踝关节骨折的疗效分析

Analysis on the Efficacy of Open Reduction and Internal Fixation in the Treatment for Ankle Joint Fracture

目的 观察切开复位内固定治疗踝关节骨折的临床疗效。方法 选择2008年4月至2012年2月武汉市武昌医院收治的踝关节骨折患者54例,均采用切开复位内固定术治疗,术后随访观察骨折愈合及患踝关节功能恢复情况。结果 所有踝关节骨折患者均获得随访,随访时间为6~12个月,平均（8.2±1.5）个月,手术伤口均为Ⅰ期甲级愈合,未见骨折不愈合、踝关节僵直、内固定螺钉断裂和创伤性关节炎等并发症,内固定取出平均时间为术后（1.0±0.2）年。根据Baird-Jackson踝关节评分标准评定疗效,优37例,良12例,可5例,优良率为90.7%。结论 切开复位内固定治疗踝关节骨折具有创伤程度轻微、骨关节复位准确、内固定牢靠、可早期行功能锻炼和疗效显著等优点,值得临床推广应用。

Objective To observe the clinical efficacy of open reduction and internal fixation in the treatment for ankle joint fracture. Methods A total of 54 patients with ankle joint fracture admitted in Wuehang Hospital from Apr. 2008 to Feb. 2012 were treated with open reduction and internal fixation. The status of fracture healing and ankle joint functional recovery was observed during postoperative follow-up. Results All the patients with ankle joint fracture were followed up, the follow-up time was 6 months to 12 months, averaged （8.2 ±1.5） months, operation wounds of all the cases were phase I and Class-A healing, there were no incidence d nonunion,stiffness, ankle fixation screw fracture and traumatic arthritis and other complications found,and the average time of the internal fLxation removal was （ 1.0± 0.2） years after operation. According to the Baird ankle joint scoring efficacy criteria,37 cases ranked excellent, 12 cases ranked good ,5 cases ranked fair, the excellent and good rate was 90.7%. Conclusion The treatment of open reduction and internal fixation for ankle joint fracture has the advantages of slight damage, accurate reset, reliable fixation, early functional exercise and good curative effect,therefore,is worthy of clinleal application.
