摘要 目的探讨脊柱损伤的MRI特点与诊断价值。方法搜集46例脊柱损伤患者,行X线、CT和MRI检查,对其三者进行对比分析。结果 X线检查46例,显示压缩性骨折58个椎体,表现为椎体变扁,不能区分新鲜与陈旧性骨折。CT检查22例,显示压缩性骨折25个椎体,其中新鲜骨折17个,陈旧性骨折4个,不能区别新旧骨折4个椎体。MR检查46例,显示新鲜骨折53个椎体,椎体呈楔形改变,边界不清,信号不均匀,T1WI呈低信号,T2WI呈等低信号,T2WI抑脂序列呈明显高信号。陈旧骨折10个椎体,椎体呈楔形改变,T1WI、T2WI均呈等低信号,T2WI抑脂序列呈等信号。显示挫伤42个椎体,椎体及附件形态正常,T1WI、T2WI均呈等或略低信号,T2WI抑脂序列呈明显高信号。结论 MRI能提示脊柱椎体损伤的病理改变,明确区别新鲜与陈旧性骨折,能准确诊断椎体挫伤,是评价脊柱损伤、指导临床治疗的最佳检查方法。 Objective To study the characteristics and diagnostic value of MRI of spinal injury. Methods Collect the X-rays, CT and MRI dates of 46 patients of spinal injuries, and analyze the difference among them. Results X-rays demonstrated 58 vertebral body compression fracture of 46 patients, characterized by variable flat vertebral body cannot differentiate between fresh and old fractures. CT demonstrated 25 compression fracture of vertebral body of 22 cases, where fresh fracture 17, of old fracture of 4, not 4 vertebral body fracture of differences between old and new. MRI displays of fresh vertebral body fracture 53 of 46 cases, wedge-shaped vertebral body changes, boundaries are not clear, the signal is not uniform, T1WI low signal, T2WI low signal, T2WI fat suppression sequence is significantly higher signal. 10 vertebral body fracture of old, wedge-shaped vertebral body changes, such as T1WI, T2WI low signal, T2WI fat suppression sequence signal. 42 shows contusion vertebral, vertebral body and attachment have normal, T1WI, T2WI or slightly low signal, T2WI fat suppression sequence is significantly higher signal. Conclusions MRI prompt the pathological changes of spinal injury, displayed the difference between the fresh and old fractures. MRI is the best way to diagnosed the spinal contusion and could be the evaluation of the treatment of spinal injuries.
