人性化护理对重症肺炎呼吸衰竭患者使用无创呼吸机的影响

Effect of humanistic nursing on the use of non-invasive ventilation in patients with severe pneumonia and respiratory failure

目的探讨人性化护理对重症肺炎呼吸衰竭患者使用无创呼吸机的影响。方法将38例确诊为重症肺炎伴呼吸衰竭的患者随机分为常规护理组和人性化护理组,常规护理组给予控制感染、吸氧、化痰、解除支气管痉挛、无创呼吸机辅助呼吸及纠正水、电解质、酸碱平衡失调、支持对症等治疗及常规护理,人性化护理组在常规护理组的基础上采用人性化护理,比较两组治疗前后血气分析结果及每天无创通气时间、病死率、气管插管率。结果治疗后,人性化护理组的pH、PaO2及SPO2显著高于治疗前及常规护理组,PaCO2显著低于治疗前及常规护理组(P<0.05)。两组的每天无创通气时间、病死率、气管插管率比较差异有统计学意义(P<0.05)。结论人性化护理可以改善缺氧,减少二氧化碳潴留,提高患者的生存率,降低其死亡率。

Objective To investigate the effect of humanistic nursing on the use of non-invasive ventilation in patients with severe pneumonia and respiratory failure. Methods 38 patients with severe pneumonia and respiratory failure were randomly divided into routine nursing group and humanistic nursing group.The control infection,oxygen,expectorant,relieve bronchial spasm,non-invasive ventilator assisted breathing and correct water, electrolyte,acid-base balance,supportive and symptomatic treatment and routine nursing care were used in routine nursing group,humanistic nursing was applied in humanistic nursing group based on routine nursing group.Blood gas analysis result before and after treat- ment,non-invasive ventilation time,survival rate,the rate of endotraeheal intubation in two groups was compared respec- tively. Results After treatment,pH,PaO 2 and SpO2 in humanistic nursing group was significantly higher than that in routine nursing group and before treatment respeetively,PaCO2 in humanistic nursing group was significantly lower than that in routine nursing group and before treatment respectively （P〈0.05）.The non-invasive ventilation time,survival rate, the rate of endotracheal intubation between two groups was compared respectively,with statistical difference （P〈0.05）. Conclusion Humanistic nursing can improve hypoxia,reduce the carbon dioxide retention,increase the survival rate of patients,reduce the mortality rate.
