动脉插管栓塞法治疗剖宫产术后晚期出血的效果观察

Effect observation of arterial embolization in treatment of late postpartum hemorrhage after cesarean section

目的 探讨动脉插管栓塞法治疗剖宫产术后晚期出血的临床效果.方法 选择2008年4月～2014年4月入住本院治疗的晚期产后出血患者64例,随机将其均分为治疗组与对照组,对照组患者实施常规保守治疗,治疗组患者给予动脉插管栓塞法,治疗3个月后进行电话随访,比较两组患者的月经恢复、不规则阴道出血、子宫保留情况及患者满意度.结果 治疗组月经恢复率高于对照组,不规则阴道出血率及子宫切除率低于对照组,差异有统计学意义（P＜0.05）.治疗组患者的满意度显著高于对照组,差异有统计学意义（P＜0.01）.结论 动脉插管栓塞法治疗剖宫产术后晚期出血,不但止血效果显著,月经恢复情况良好,而且能最大程度地保留患者的子宫,值得临床推广.

Objective To investigate the clinical effect of arterial embolization in the treatment of late postpartum hemorrhage after cesarean section. Methods 64 cases of patients with late postpartum hemorrhage treated in our hospital from April 2008 to April 2014 were selected and randomly divided into treatment group and control group,the control group were given with conventional conservative treatment,the treatment group were given with arterial embolization, telephone follow-up for 3 months after treatment,the menstruation,irregular vaginal bleeding,uterine reservation and patient satisfaction were compared between the two groups. Results The menstruation rate in the treatment group was higher than that of the control group,the rate of irregular vaginal bleeding and uterine reservation were lower than those of control group,the differences were significant （P〈0.05）.The patient satisfaction of treatment group was higher than that of the control group,the difference was significant （P〈0.01）. Conclusion Arterial embolization for treating late postpartum hemorrhage after cesarean section,not only has significant hemostatic effect,menstrual recovery in good condition,but also can furthest reserve the patient＇s uterus,is worthy of promotion in clinic.
