帕金森病患者睡眠障碍的临床特征及影响因素分析

Analysis of Clinical Features and Influential Factors of Sleep Disorders in Patients with Parkinson＇s Disease

[目的]探讨帕金森病（PD）睡眠障碍发生的相关临床因素.[方法]选取本院门诊及住院的PD患者76例,记录所有PD患者的年龄、性别、使用多巴丝肼剂量、受教育程度,并对所有PD患者行帕金森病评价量表（UPDRS）、Hoehn Yahr（H-Y）分期、疲劳严重度量表（FSS）评分、汉密尔顿抑郁（HAMD）评分、匹兹堡睡眠质量指数评分（PSQI）、爱泼沃斯嗜睡量表（ESS）等评分,根据患者有无睡眠障碍将PD患者分为睡眠障碍组与非睡眠障碍组,分别比较两组患者的临床资料并利用Spearman分析探讨PD患者睡眠障碍发生的相关因素.[结果]76例PD患者中,48例存在睡眠障碍,发生率为63.2％,PD睡眠障碍组患者UPDRS评分、H-Y分期、FSS评分、多巴丝肼剂量、HAMD评分明显高于非障碍组患者,且两组相比较差异均有显著性（均P ＜0.05）.PD患者睡眠障碍的发生与UPDRS评分、H-Y分期,FSS评分、多巴丝肼剂量、HAMD评分有关（r =0.56～0.81,均P＜0.05）,而与性别、年龄、病程、受教育程度无关.[结论]PD患者的睡眠障碍发生率较高,影响因素较多,值得在临床工作中加以重视.

[Objective]To explore the related factors of sleep disorders in patients with Parkinson＇s disease （PD）.[Methods] A total of 76 patients with PD in our hospital were chosen.The age,gender,dose of Madopar and education level of patients with PD were recorded.In addition,unified Parkinson＇s disease rating scale （UPDRS）,Hoehn-Yahr（H-Y） stage,fatigue severity scale（FSS） score,Hamilton depression scale（HAMD） score,Pittsburgh sleep quality index（PSQI） and Epworth sleepiness scale （ESS） were used in PD patients.According to with or without sleep disorder,all patients with PD were divided into sleep disorder group and non-sleep disorder group.Clinical data were analyzed and compared between two groups.Spearman analysis was used to discuss the related factors of sleep disorder in PD patients.[Results] Among 76 patients,48 patients had sleep disorders,and the incidence rate was 63.2％.UPDRS scores,H-Y stage,FSS scores,Madopar dosage and depression scores in PD with sleep disorder group were obviously higher than those in PD without sleep disorder group,and there were significant differences between two groups（all P ＜0.05）.Sleep disorder in PD patients was associated with UPDRS,H-Y stage,FSS scores,Madopar dosage and depression scores（r =0.56～0.81,all P ＜0.05）,but not correlated with gender,age,disease duration and education level.[Conclusion] The incidence rate of sleep disorders in PD patient is high and has various influential factors.Therefore,it should be emphasized in clinical practice.
