改良剥脱法治疗下肢静脉曲张的临床研究

Clinicalstudies of modified stripping for varicose veins of lower extremity

目的研究传统静脉剥脱器增加自制丝线性材料剥离头改良剥脱法治疗下肢静脉曲张的临床效果。方法以2012年5月-2013年5月到鞍钢总医院行手术治疗的原发性单纯性下肢大隐静脉曲张的46例患者（46条下肢）为研究对象,按照入院顺序分为观察组24例,采用传统静脉剥脱器增加自制丝线性材料剥离头改良剥脱法剥脱大隐静脉主干;对照组22例,采用传统静脉剥脱器剥脱法剥脱大隐静脉主干;比较两组患者手术时间、切口数（≥1cm）、术中出血量、术后24h肢体的疼痛分级、术后疼痛持续时间、皮下淤血及肢体肿胀麻木患者例数、术后住院时间等指标。结果改良剥脱法治疗大隐静脉曲张较传统剥脱器剥脱法手术时间缩短,术中出血量减少,切口数减少,一次性剥脱成功率高（95.8%）,术后疼痛时间低于传统剥脱器剥脱法术,差异有统计学意义（P〉0.05）。结论改良剥脱法治疗下肢静脉曲张的临床效果优于传统静脉剥脱器。

Objective To study clinical effect for varicose veins with modified stripping by traditional vein stripping with homemade silk material peeling head. Methods From May 2012 to May 2013,46 patients（46 lower limbs）with primary varicose great saphenous were divided into observation group（24 cases）using the traditional vein stripping with homemade silk material peeling head; control group（22 cases）using the traditional vein stripping.Surgical time,incision number（≥1 cm）,blood loss,limb pain level in 24 h postoperative,duration of postoperative pain, number of skin bruising, swelling and numbness of limbs, postoperative hospital stay were compared.Results Shorter operation time, reduced blood loss,decreased incision number, higher success rate of one-time denudation （95.8%）, shorter duration of postoperative pain were showed in observation group than in control group,and the difference was statistically significant（P〈0.05）.Conclusion Modified stripping is superior to the traditional venous stripper for varicose veins of lower extremity.
