我院2011年3-7月门诊不合理处方分析

Investigations and Analyses of Irrational Outpatient Prescriptions in the Hospital in March to July 2011

目的:了解我院门诊处方用药情况,对不合理处方进行分析,以促进医院门诊合理用药水平。方法:采用回顾性调查方法,收集我院2011年3-7月门诊全部处方465 941张(静脉用药处方除外),参照《处方管理办法》对其进行审核,对其中的不合理处方进行统计分析。结果:465 941张处方中,不合理处方5 237张,占1.12%,其主要不合理项目为用法用量不适宜(59.25%)、前记不完整(15.52%)、单张处方超过5种药品(9.01%)、无医师签名(4.93%)、用药无指征(4.20%)、药品规格书写不规范(2.71%)、修改处无签章或签名(2.41%)、选药不适宜(1.60%)、联合用药不适宜(0.23%)等。结论:我院门诊处方存在一定程度的不合理问题,医院管理部门应完善监管机制,医师、药师相互配合,以促进合理用药,保障患儿用药安全。

Objective： To study the situation about the administration of prescription drugs in the outpatient department of the hospital, and to promote the level of rational administration of drugs. Methods： On the basis of Prescription Management Methods, adapt the method of retrospective survey to examine 465,941 prescriptions （excluding the prescriptions of intravenous drugs） which were collected from the hospital in March to July 2011, and statistically analyze the irrational prescriptions. Results： The 5,237 prescriptions which account for 1.12% of the total were irrational. In irrational prescriptions, unsuited for usage and dosage prescriptions were 59.25 %, the incomplete information record （15.52%）, single prescription for more than five kinds of medicines （9.01%）, and no physician signature （4.93%）, no indications of use drug （4.20%）, non-standard specifications written （2.71%）, modified without signature or sign （2.41% ）, drug choice is not appropriate （ 1.60% ）, unsuited combination （0.23 % ）. Conclusion ： The percentage of irrational outpatient prescriptions was high in our hospital. Effective measures should be made to improve the quality of prescriptions and standardize prescriptions regulation.
