热带地区急性缺血性中风中医证型与脑梗死DWI影像ADC值关系研究

Correlation of TCM syndromes of acute ischemic stroke in the tropics with cerebral DWI images ADC

目的：探讨热带地区急性缺血性中风中医证型与脑梗死DWI影像ADC值间的关系.方法：回顾性收集2010年8月～2013年12月经三亚市中医院临床及MR-DWI确诊的急性缺血性中风患者138例（包括动脉粥样硬化性血栓性脑梗死,腔隙性脑梗死）,按1993年颁布的《中药新药临床研究指导原则》将其以中经络组分为：肝阳暴亢、风火上扰;风痰瘀血、痹阻脉络;痰热腑实、风痰上扰;气虚血瘀;阴虚风动.分析其脑梗死DWI图像资料,并测量病灶的ADC值等信息.根据试验数据,应用SPSS13.0统计软件对其中128例资料完整者进行统计分析.结果：由于中经络组中的第-证和第五症样本量过小,进行了删除.作三种分型间的方差分析及非参数检验,结论-致,即容积的组间存在差异,而ADC值不存在差别.本数据不完全满足正态性,适宜做非参数检验,（Kruskal-WallisTest）,P=0.183,差别没有统计学意义.结论：热带地区的不同急性缺血性中风中经络组中医证型之间ADC值的差异未见特异性,ADC值作为中医分型的客观指标有待我们下-步的大样本研究来探讨.

Objective:To investigate the correlation of TCM syndromes of acute ischemic stroke in the tropics with cerebral DWI images ADC. Methods: 138 cases (including atherosclerotic thrombotic infarction, lacunar infarction) diagnosed with acute ischemic stroke by Sanya City Hospital clinical and MR-DWI from August 2010 -December2013 were retrospectively collected and divided into liver yang hyperactivity, wind&Fire disturbance;wind phlegm blood stasis, blocking the meridians;phlegm and heat up interference;Qi deficiency and blood stasis;Yin deficiency and wind movement. according to TCM new drug clinical research guidelines issued in 1993.The cerebral DWI image data and measurement of lesion ADC values and other informationwas analyzed,. Based on test data, application SPSS13.0 statistical software which 128 cases with complete data for statistical analysis. Results:Due to the Meridian group first card and fifth disease sample size was too small, they were removed. For analysis of variance and non-parametric tests between the three sub-type, the same conclusion, that there was a difference between the volume of the group, but the difference between ADC values did not exist. This data did not fully meet normality, suitable as a non-parametric test, (Kruskal-Wallis Test), P=0.183, the difference was not statistically significant. Conclusion:Differences in acute ischemic stroke in the tropics conclusion ADC values between TCM meridian group are not special, ADC value as an objective indicator of TCM genotyping large sample needs to be discussed.
