头臂动脉支架成形术中基底动脉栓塞一例并文献复习

摘要 患者男,74岁,因发作性头晕伴左侧肢体活动不灵5个月,加重2周于2013年8月27日入院.患者5个月前无明显诱因出现头晕,伴左侧肢体活动不灵,于外院诊断为“脑血栓形成”,予保守治疗后症状缓解,近2周症状再发并较前加重,为求进一步诊治来我院.头颅CT示右侧基底节区陈旧梗死灶,脑内多发腔隙性梗死;头颈部CT血管成像（CTA）示右颈内动脉及头臂动脉起始部均重度狭窄（＞80％）（图1）.经改善脑循环、抗血小板、调整血脂对症治疗,患者症状无缓解,科室讨论后于2013年8月30日局部麻醉下行右颈内动脉＋头臂动脉支架成形术.
