急性缺血性脑卒中后高血糖的研究进展 被引量:14

Recent advance in hyperglycemia after acute ischemic stroke

摘要 脑卒中是我国目前第一大死亡病因及成年人首位致残原因[1].缺血性脑卒中占所有脑卒中的70%~80%,是严重危害人类健康的致死性和致残性疾病.糖尿病患病率目前在我国呈逐年上升趋势,糖尿病引起的慢性并发症,尤其大、中血管并发症,是严重影响患者生活质量和缩短患者寿命的重要原因.近年来大量研究表明,高血糖与缺血性脑卒中之间存在密切的内在联系:一方面,高血糖是缺血性脑卒中的独立危险因素,高血糖患者发生缺血性脑卒中的风险高于血糖正常者2倍以上,且该类患者在缺血性脑卒中发作后的临床表现、脑梗死体积、病死率、致残率及脑卒中复发率等方面也均更为显著[2-3];另一方面,缺血性脑卒中患者中糖代谢异常的发生率亦显著高于一般人群,且脑卒中本身所导致的应激反应可同时干扰糖代谢、诱导血糖升高.
