CT在血友病髂腰肌血肿误诊中的应用价值

Value of computed tomography in reducing the misdiagnosis rate of hemophilia iliopsoas bleeding

目的探讨CT在血友病髂腰肌出血误诊中的应用价值。方法收集血友病髂腰肌出血患者资料17例,全部病例均行1次或多次CT检查,对比这些患者的初步诊断与CT诊断间的差异,通过CT表现及临床表现分析形成差异的原因。结果17例患者经CT检查,均确定髂腰肌血肿的诊断,6例与初步诊断不符,占35%。初步诊断误诊为髋关节出血2例,腹腔出血1例,阑尾炎1例;臀部肌肉出血1例,大腿肌肉出血1例。结论 CT检查可以明确血友病髂腰肌出血的诊断、排外其他髂腰肌病变鉴别,可与周边其他肌肉或脏器出血相鉴别。

Objective To explore the value of computed tomography（CT） in reducing the misdiagnosis rate of hemophilia iliop-soas bleeding. Methods 17 hemophilia patients with Hemophilic iliopsoas hematoma were enrolled,All patients underwent one or more times plain CT scan. Contrast the difference between initial diagnosis and CT diagnosis ,analysis the reasons for the formation differences by CT and clinical performance. Results All patients were confirmed the diagnosis of iliopsoas muscle hematoma after CT scanning. including 6 cases （35%） of initial diagnosis errors. 2 patients were diagnosed with hip bleeding in the primary diag-nosis. One patient was diagnosed with intraperitoneal hemorrhage,1 patient was diagnosed appendicitis. 1 patient was diagnosed with coxal muscle bleeding 1 patient was diagnosed with a thigh muscle bleeding. Conclusion CT scanning can clearly diagnose iliopsoas bleeding. Excluded other iliopsoas diseases,identify the surrounding muscle or other organs bleeding.
