丹红注射液对全髋关节置换术患者血小板及血流变状态的影响

Influence of Danhong injection on the platelets and rheology status of patients with total hip arthroplasty

目的:观察丹红注射液对全髋关节置换术患者血小板及血流变状态的影响。方法:选取120例进行全髋关节置换术的患者,随机分为对照组和观察组各60例。对照组采用基础治疗,观察组则在术后静脉滴注丹红注射液。比较治疗前后两组间血小板活化相关指标、凝血指标及血液流变学指标差异。结果:治疗后第7、14天,观察组血小板活化相关指标血清血小板膜糖蛋白Ⅱb/Ⅲa(GPⅡb/Ⅲa)、P-选择素、血小板膜糖蛋白CD62P(CD62P)水平明显低于对照组,差异具有统计学意义(P<0.05)。观察组活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、凝血酶原时间(PT)、纤维蛋白原(Fb)明显长于对照组,血清D-D水平明显低于对照组,而血清Fig水平则高于对照组,差异具有统计学意义(P<0.05);观察组全血高切黏度、全血低切黏度、全血中切粘度、血浆粘度、红细胞聚集指数均明显低于对照组,差异具有统计学意义(P<0.05);观察组治愈率明显高于对照组,差异具有统计学意义(P<0.05)。结论:丹红注射液有效抑制全髋关节置换术后血小板活化,改善血液高凝状态,从而有效抑制深静脉血栓的形成。

Objective： To observe the influence of Danhong injection on the platelets and rheology status of patients with total hip arthroplasty. Methods： A total of 120 patients with total hip arthroplasty were randomly divided into control group and observation group, with 60 cases in each group,patients in control group were given basis treatment, while those in obser vation group were given Danhong injection after undergoing surgery. Then the indexes of platelet activation, coagulation and theology of two groups before and after treatment were compared. Results： After 7, 21 days of treatment, the serum levels of indexes of platelet activation such as GP Ⅱ b / Ⅲ a, P selectin, CD62P of observation group were all lower than control group; and the indexes of coagulation such as APTT, TT, PT, Fb of observation group were all longer than control group, the serum level of D-D was significantly lower than the control group, while the Fig level was higher than control group; the indexes of rheology such as high shear whole blood viscosity, low shear whole blood viscosity, shear whole blood viscosity, plasma viscosity, erythrocyte aggregation index of observation group were all significantly lower than control group （P〈 0.05）. Conclusion：Danhong injection can effectively inhibit platelet activation for patients with total hip arthroplasty, improve the hypercoagulable states, thus effectively inhibit the formation of deep vein thrombosis.
