摘要 目的:探讨血清脂蛋白(a)[Lp(a)]、视黄醇结合蛋白(RBP)和同型半胱氨酸(Hcy)联合检测在高血压肾病(HRD)早期肾损害诊断中的临床应用价值.方法:对我院2013年1月-2013年12月收治的83例HRD患者(HRD Ⅰ组27例,HRDⅡ组32例,HRDⅢ组24例)及30名健康体检者(对照组)进行血清Lp(a)、RBP和Hcy检测,同时检测与肾功能密切相关的血清学指标尿素(UREA)和肌酐(CREA),并按患者高血压病的病程进行分组,对检测结果进行比较分析.结果:各组HRD血清Lp(a)、RBP、Hcy、UREA和CREA高于对照组(P<0.05或P<0.01);HRD各组三项指标的阳性检出率高于常规血清学检测指标UREA、CREA (P<0.05).结论:血清Lp(a)、RBP和Hcy可作为HRD早期肾损害诊断的敏感指标,三项指标联合检测可提高HRD的阳性检出率,为HRD早期治疗、疗效观察、病情监测及预后等方面提供有价值的实验室依据. Objective:The clinical value of serum lipoprotein (a) (LP) (a), retinol binding protein (RBP) and homocysteine (Hcy) in the combined testing of renal damage of the diagnosis in early of hypertensive nephropathy (HRD). Mothods : 83 HRD patients in our hospital from January to December in 2013 (HRD I group 27 cases, HRD II group 32 cases, HRD III group 24 cases) and 30 cases healthy persons (control group) of serum LP (a), Pd3P and Hcy testing, at the same time, serum urea testing and renal function closely related (UREA) and creatinine (CREA), and were divided into two groups according to hypertension disease, carried on the comparative analysis of test results.Results : The serum HRD LP (a), RBP, Hcy, UREA and CREA higher than that of the control group (P 〈 0.05 or P 〈 0.01); three indicators HRD grouP Positive rate is higher than that of conventional serological indexes UREA, CREA (P 〈 0.05). Conclusions : The serum LP (a), RBP and Hcy can be used as a sensitive index for the diagnosis of HRD in early renal damage, three combined testing indexes can improve the positive rate of HRD, providing valuable laboratory basis for the early treatment of HRD, observation of curative effect, disease monitoring and prognosis.
