摘要 目的:研究糖尿病患者尿液中白蛋白尿指标对于疾病的反映程度.方法:选取2011年8月至2013年12月泰安市某中医院临床确诊的男性2型糖尿病患者,所有患者符合2007年美国糖尿病学会(ADA)制订的《糖尿病临床实用指南》中糖尿病诊断标准.依据尿蛋白水平分为3组,研究白蛋白尿指标的变化.结果:本研究发现,各组患者的尿液代谢产物水平呈现完全不同的分布.其中,尿白蛋白正常组可能包括单纯DN(糖尿病肾病)以及Mogensen(糖尿病肾病疾病)分期中Ⅰ期和Ⅱ期DN患者.结论:据此发现疾病早期诊断和进展的候选标志物.进一步证实尿白蛋白正常组并非一个均一整体,部分尿白蛋白正常患者与微量白蛋白尿患者代谢水平差异不大,可能已存在肾脏损伤. Objective : to study the albuminuria in diabetic urine indexes to reflect the degree of disease. Methods : select in August 2010 to December 2012 institute of a Xuzhou traditional Chinese medical hospital of kidney disease clinical diagnosis in patients with type 2 diabetes, all conform to the American diabetes association in 2007 patients with (ADA) in the formulation of the diabetes clinical practicalguide diabetes diagnostic criteria. On the basis of urine protein levels are divided into three groups, the index changes in albuminuria.Results ; This study found that groups of the patient's level of urine metabolites present completely different distribution. Among them, the normal urinary albumin group may include pure DM and Mogensen staging phase I and phase II in DN patients, its stage with microalbuminuria and clinical diabetic nepliropathy among three groups of patients in clinical indicators and it exist significant differences in metabolism prompt as the disease progresses, the DN abnormal metabolism in patients with continuous and does not have different metabolic characteristics about the 'at the same time period. Conclusions : On the basis of spot disease early diagnosis and progression of candidate markers. At the same time, we also observed PLS - DA urinary albumin in the model the microalbuminuria group and normal group of a small amount of overlap between urine samples, and the model for normal urinary albumin group and microalbuminuria group differences explain and predict degrees are lower than the rest of the group, which further confirmed the normal urinary albumin group is not a uniform whole, part of the urinary albumin in patients with normal metabolism of patients with microalbuminuria level difference, kidney damage may already exists. Therefore, microalbuminuria as DN early diagnosis index lower sensitivity.
