摘要
目的研究不同波形和频率的电针对神经根型颈椎病的临床疗效。方法 240例神经根型颈椎病患者按不同电针刺激参数组合随机分为A组、B组、C组、D组、E组和F组,每组40例。6组患者均采用电针治疗,A组参数为连续波4 Hz,B组为连续波40 Hz,C组为疏密波l4次/min,D组为疏密波20次/min,E组为断续波l4次/min,F组为断续波20次/min。疗程结束后,观察各组治疗前后症状体征积分、VAS评分的改善情况及临床疗效。结果各组治疗后症状体征积分和VAS评分与同组治疗前比较,差异均具有统计学意义(P<0.01)。A组、B组、C组和D组治疗后症状体征积分和VAS评分与E组和F组比较,差异均具有统计学意义(P<0.01)。A组总有效率为92.5%,B组为95.0%,C组为90.0%,D组为87.5%,E组为75.0%,F组为70.0%。各组间总有效率经卡方检验,差异具有统计学意义(P<0.01)。结论高频连续波应该作为治疗神经根型颈椎病的首选波形。
Objective To investigate the clinical efficacies of electroacupuncture with different waveforms and frequencies in treating cervical spondylotic radiculopathy. Methods Two hundred and forty patients with cervical spondylotic radiculopathy were allocated, according to different electroacupuncmre stimulation parameters, to groups A, B, C, D, E and F, 40 cases each. All the six groups received electroacupuncture treatment. The parameters were continuous waves and a frequency of 4 Hz in group A, continuous waves and a frequency of 40 Hz in group B, sparse-dense waves and a frequency of 14 times/rain in group C, sparse-dense waves and a frequency of 20 times/min in group D, interrupted waves and a frequency of 14 times/min in group E, and interrupted waves and a frequency of 20 times/min in group F. In every group, the symptom and sign score and the VAS score were counted before treatment and after the end of it and the clinical therapeutic effect was evaluated after the end of treatment. Results There were statistically significant pre-/post-treatment differences in the symptom and sign score and the VAS score in every group (P〈0.01). There were statistically significant post-treatment differences in the symptom and sign score and the VAS score between group A, B, C or D and group E or F (P〈0.01). The total efficacy rate was 92.5% in group A, 95.0% in group B, 90.0% in group C, 87.5% in group D, 75.0% in group E and 70.0% in group F. Chi-square test showed a statistically significant difference in the total efficacy rate among the groups (P〈0.05). Conclusion Continuous waves with a high frequency should be the first choice of waveform for the treatment of cervical spondylotic radiculopathy.
