采用调查问卷评价帕金森病患者生活质量及影响因素研究

Assessment of quality of life of patients with Parkinson's disease with questionnaire and analysis of its influencing factors

目的采用EuroQol五维调查问卷(EQ-5D)对帕金森病患者的生活质量进行评价,并分析其影响因素。方法选择帕金森病患者52例,采用EQ-5D、统一帕金森病评定量表(unified Parkinsons diseasc rating scale,UPDRS)、Hoehn-Yahr(H-Y)分级、帕金森病睡眠量表(Parkinsons disease sleep scale,PDSS)、汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA),对帕金森病患者的生活质量和健康状况进行分析。结果帕金森病各评定量表间相关分析显示,失眠(r=-0.519)、UPDRSⅠ(r=-0.568)、UPDRSⅡ(r=-0.705)、UPDRSⅢ(r=-0.760)、UPDRSⅣ(r=-0.571)、HAMD(r=-0.669)、HAMA(r=-0.549)、H-Y分级(r=-0.694)、PDSS(r=1.000)、病程(r=-0.640)、文化程度(r=0.293)与帕金森病患者生活质量有关(P<0.01)。多元回归分析显示,影响EQ-5D指数因素有UPDRSⅠ、UPDRSⅢ、HAMD、HAMA和PDSS,其中UPDRSⅠ、UPDRSⅢ、HAMA和HAMD与EQ-5D指数呈负相关(P<0.05,P<0.01);PDSS与EQ-5D指数呈正相关(P<0.01)。结论 EQ-5D适用于帕金森病患者生活质量研究,临床医师在重视帕金森病患者的诊断和运动症状治疗前提下,还应重视患者睡眠、焦虑抑郁等非运动症状,尽量提高患者生活质量。

Objective To assess the quality of life of patients with Parkinson＇s disease（PD）with EQ-5Dquestionnaire and analyze its influencing factors.Methods Fifty-two PD patients were included in this study.Their quality of life and health status were assessed according to the EQ-5D,UPDRS,Hoehn-Yahr（H-Y）stage,PD sleep scale（PDSS）,Hamilton anxiety rating scale（HAMA）amd Hamilton depression rating scale（HAMD）.Results Insomnia（r=-0.519）,UPDRSⅠ-Ⅳ（r=-0.568,r=-0.705,r=-0.760,r=-0.571）,HAMD（r=-0.669）,HAMA（r=-0.549）,H-Y stage（r=-0.694）,PDSS（r=1.000）,disease course（r=-0.640）and education level（r=0.293）were related with the quality of life of PD patients（P〈0.01）.Multivariate regression analysis showed that the factors that influenced the EQ-5Dindex of PD patients were UPDRSⅠ,UPDRSⅢ,HAMD and PDSS.The UPDRSⅠ,UPDRSⅢ,HAMA and HAMD were negatively related with the EQ-5Dindex（P〈0.05,P〈0.01）,while the PDSS was positively related with EQ-5Dindex（P〈0.01）.Conclusion EQ-5Dcan used in assessment of the quality of life of PD patients.Clinicians should pay their attention not only to the diagnosis of PD patients and treatment of their motor symptoms,but also to their non-motor symptoms such as sleep,anxiety and depression in order to improve their quality of life.
