应用SPECT研究头皮穴区局部浸润麻醉后对电针信号传导的影响 被引量:19

Study on Effects of Local Infiltration Anesthesia of the Scalp Point Area on Conduction of Electroacupuncture Signs by Means of SPECT

摘要 目的 :探讨和验证腧穴和神经系统的关系。方法 :选择头皮运动区上 1/ 5和中 2 / 5处针刺 ,观察 1小时之内连续接受针刺前、电针和局部浸润麻醉后电针刺激 3次SPECT脑灌注显像。结果 :针刺前见双侧大脑皮层、丘脑、基底节和小脑的血流灌注和功能基本对称 ;当电针头皮运动区时 ,以对侧丘脑、对侧额叶、对侧顶叶的运动和感觉皮质区增高更为明显 ;而当局部浸润麻醉后再电针时 ,对侧丘脑血流灌注和功能下降。定量分析结果和视觉所见相同。 结论 :①一侧头皮针刺激可以同时改善双侧局部脑血流灌注和功能活动 ;②研究结果支持头皮针刺信号是通过三叉神经传导径路入脑并发挥效应 ;③依据SPECT研究结果尚看不出头穴分区与大脑皮层各功能区存在相应的定位关系 ;④当穴区被局部浸润麻醉后 。 Purpose To explore and test and verify the relation between acupoints and the nervous system. Methods Acupuncture was given at upper 1/5 and middle 2/5 of the motor area of scalp, and each patient received 3 times of SPECT cerebral perfusion development before acupuncture, during electroacupuncture and electroacupuncture stimulation after local infiltration anesthesia within 1 hour. Results Before acupuncture the blood perfusion and the function at bilateral cerebral cortex, thalamus, basical ganglion and cerebellum were basically symmetric; at electroacupuncture of the motor area, the motor and sensor cortex areas of contralateral thalamus, frontal lobe and parietal lobe increased more obviously; acupuncture after local infiltration anesthesia decreased the blood perfusion and function of contralateral thalamus. Quantitative analysis and finding of vision had same results. Conclusion ①Stimulation of one side of the scalp can improve simultaneously bilateral local cerebral blood perfusion and functional activity. ②The signs of acupuncture at scalp enter the brain to exert effects through transmitting pathway of the trigeminal nerve. ③It is not found by SPECT that there is corresponding location relation between scalp regions and various functional areas of the cerebral cortex. ④Effects of acupuncture signs are reduced obviously or blocked after local infiltration anesthesia of the scalp point area.
