摘要
目的:评估18F-FDG PET/CT在寻找多发性骨转移瘤原发灶中的临床价值。方法:回顾性分析136例多发性骨转移瘤患者的18F-FDG PET/CT检查资料,并与其病理结果相对照。结果:136例患者中,经18F-FDG PET/CT发现原发灶者共85例,其中79例经手术或活检病理证实为原发灶,另有6例证实为假阳性。51例多发性骨转移瘤未找到原发灶,其中3例在随访中证实为假阴性,其余48例在随后的随访中仍未发现原发灶。利用18F-FDG PET/CT寻找多发性骨转移瘤原发灶的灵敏度为96.3%,特异度为88.9%,符合率为93.4%。有79例患者找到了原发病灶,检出率为58.1%(79/136)。结论:应用18F-FDG PET/CT寻找多发性骨转移瘤的原发灶有一定临床价值,对制定合适的个性化治疗方案有积极作用。
Objective To evaluate the value of 18F-FDG PET/CT for detecting the primary lesion of patients with multiple bone metastases. Methods mSF-FDG PET/CT were performed in 136 patients with multiple bone metastasis, the results were then compared with the pathologic diagnosis. Results Primary lesion was detected by 18F-FDG PET/CT in 85 patients, and 79 of them were confirmed pathologically, and the other 6 patients were false-positive. Fifty-one patients were fail to detect the primary lesion, and 3 of them were confirmed as false-negative by follow-up. The sensitivity of 18F-FDG PET/CT for detecting primary lesion was 96.3%, the specificity was 88.9%, and the accuracy rate was 93.4%. The overall detection rate of primary lesion was 58.1% (79/136). Conclusions 18F-FDG PET/CT has substantial value for detecting primary lesion in patients with multiple bone metastases, and could provide significant help for formulating appropriate individualized therapy regimen.
