214例重症心脏瓣膜病合并巨大左室的外科治疗

Heart valve replacement for giant left ventricle:clinical analysis of 214 cases

心脏瓣膜病合并左室舒张末期内径〉70mm的巨大左心室患者,是瓣膜病手术的高危病例。随着近年心肌保护及围手术期处理技术的发展,此类病例的治疗效果已明显提高,但此类患者死亡率相对较高、并发症较多,目前仍是临床上治疗的难点。现总结我院2001-01-2013-08外科治疗214例合并巨大左心室的重症心脏瓣膜病患者围术期管理经验,报道如下。

Summary To summarize the experiences in valve replacement surgery for patients with giant left ventricle. The clinical data of 214 cases of giant left ventricle treated by valve replacement were retrospectively analyzed. The key to the efficiency of valve replacement for giant left ventricle was heart function support,management of ven- tricular arrhythmia and preserving the posterior valve of the mitral valve during perioperative period.
