摘要
目的检测系统性红斑狼疮(SLE)患者外周血滤泡辅助性T细胞(Tfh)的水平,探讨Tfh与SLE的发生及病情活动性的关系。方法 45例SLE患者和30例健康对照者,采用流式细胞术检测PBMC中Tfh水平,ELISA法检测血清中IL-21,IL-17水平,实时荧光定量RT-PCR检测PBMC中CXCR5 mRNA的表达。结果活动组SLE患者外周血Tfh,CXCR5 mRNA的表达和血清中IL-21,IL-17水平均高于非活动组和健康对照组;SLE患者外周血Tfh水平与CXCR5 mRNA的表达呈正相关,与SLEDAI亦呈正相关,与抗核抗体滴度及补体C4水平无明显相关性,与补体C3水平呈负相关。其中CXCR5 mRNA的表达水平与SLEDAI呈高度的正相关(r=0.92,P<0.01)。结论 SLE患者外周血中Tfh细胞水平明显升高,可能在其发病过程中起了一定的作用。
Objective Detection of SLE patients with the percentage of Tfh cells in peripheral blood and levels of IL-21 and IL-17 in serum were operated to investigate the relationship between the Tfh cells and SLE. Methods Venous blood was collected from 45 patients as well as 30 healthy human controls. Flow cytometry was used to detect the percentage of Tfh in peripheral blood mononuclear cells( PBMCs),enzyme linked immunosorbent assay(ELISA)was performed to measure the levels of serum IL-21 and IL-17 and rea L-time quantitative reverse transcription( RT)-PCR was operated to detect the mRNA expressions of CXCR5. Results The percentage of Tfh cells in the peripheral blood,the level of CXCR5 mRNA expression,and the serum levels of IL-21 and IL-17 were significantly higher in active SLE patients than in non-active patients and healthy human controls. The percentage of Tfh cells of SLE patients was related with the levels of CXCR5 mRNA,SLEDAI and C3 expression,while there was no correlation between the ANA titer,C4 levels,and above. The expression of CXCR5 mRNA was obviously related with SLEDAI( r = 0. 92,P 〈 0. 01). Conclusion There is a significant increase of the percentage of Tfh in SLE patients,hinting a certain role of Tfh in the pathogenesis of SLE.
