摘要 目的探讨湿疹患者食物过敏原检测的意义。方法对753例患者进行食物过敏原特异性Ig E与Ig G检测,并进行统计学分析;同时对花生、黄豆、牛奶等过敏原的特异性Ig E与Ig G不同结果组进行双盲安慰剂食物激发试验。结果食物过敏原特异性Ig E阳性检出率为40.11%,其中腰果、花生、黄豆阳性率较高;Ig G阳性检出率为79.68%,牛奶、鸡蛋、虾蟹为主要过敏原,双盲安慰剂食物激发试验证实两种抗体检测都有假阳性与假阴性。结论临床医生应在湿疹的治疗中注意筛查食物过敏原,特别是Ig G介导的过敏反应,其可为湿疹的诊疗提供更有力的依据。 Objective To explore the significance of food specific antibody detection in patients with Eczema. Methods To detect the specific food Ig E and IgG of 753 patients and 100 normal controls. Meanwhile,double-blind placebo-control food challenge( DBPCFC) was carried out with different IgE and IgG results. Results The positive rate of specific IgE is 40. 11%,and peanut、milk and soy bean had higher positive rate; the positive rate of specific IgG was 79. 68%,with milk、egg、shrimp and crab as major allergens. DCPCFC confirmed that both antibodies detection had false positive and false negative rate. Conclusion The clinicians should pay attention to the food allergen detection in patients of eczema,especially allergic response mediated by IgG.
