摘要 目的探讨D抗原表达强弱与RhCE抗原的关系。方法采用微量板法对RhD阳性献血者检测Rh表型;抗球蛋白卡式法初筛RhD阴性标本进行确认并分型;吸收放散试验对经抗球蛋白法确认为RhD阴性标本进行Del表型筛选。结果 D抗原在Rh表型不同时与抗体的凝集强度的平均值有差别,测得的D抗原凝集强度从强到弱依次为DccEE>DCCee>DccEe>DCcEe>DCcee,各表型之间抗原凝集强度存具有统计学意义(F=2.945,P<0.05);不同D表现型的RhCE表型构成是不同的,具体表现为一般D阳性与RhD阴性相比,前者C、E抗原所占比例较大,后者c抗原所占比例较大,弱D和Del型这2种特殊D阳性则表现在,C、c、E抗原比例介于两者之间。结论RHCE基因形成的抗原组合影响RHD基因表达D抗原,Cc,cc抗原组合的标本可能出现D凝集强度减弱或者极弱形成弱D或者Del型,甚至D抗原不表达形成D阴性。 Objective To analyze the relationship between the expression intensity of RhD antigen and CE antigen. Methods Microplate method was used in 448 cass of RhD positive blood donors for Rh phenotype in 2013 October; RhD negative specimens were identified and categorized with the use of the Coombs test; the absorption elution technique was used on the RhD negative specimens obtained by the Coombs Test to screen for the D11 phenotype. Results The average value of D antigen differed from that of the antibody agglutination intensity for different Rhesus phenotype. The agglutination intensities of D antigen were arranged in the descending order of strength: DecEE 〉 DCCee 〉 DceEe 〉 DCcEe 〉 DCeee. Different phenotype showed significant differences in the antigen agglutination intensities ( F = 2. 945,P 〈 0. 05 ) ; Different D phenotypeled to different RhCE phenotype formation. When comparing D positive antigen with RhD negative antigen, the former consisted of a larger proportion of C and E antigen,the latter consisted of a larger proportion of e antigen. The differences between the two specific D positive types, weak D and Del type, were found in the C, c and E antigen ratio. Conclusion The antigenic combination formed by the RttCE gene affected the RHD gene expression of D antigen. The specimen containing the combination of Cc and ce antigens could possibly result in the weakening of the D antigen agglutination intensity. The extremely weak agglutination intensity might form weak D or Del type. Weakening of the D antigen agglutination imensity might even lead to null expression of D antigens,ending in the formation of negative D phenotype.
