SWAN序列在脑静脉畸形诊断中的应用

摘要 目的探讨T2加权血管成像(SWAN)序列在脑静脉畸形(CVM)诊断中的应用价值。方法回顾性分析15例CVM患者的MRI资料,所有患者均行常规平扫、SWAN和增强扫描。结果 15例CVM中,11例位于幕上,4例位于幕下,MRI平扫显示引流静脉呈长T1短T2流空信号12例,呈长T1长T2信号3例,髓静脉均呈长T1长T2信号。SWAN清晰显示引流静脉及髓静脉呈低信号,表现为典型"海蛇头"征。增强扫描显示多条髓静脉呈放射状共同汇入引流静脉。结论 SWAN较常规MRI平扫及增强扫描能更清晰及敏感地显示CVM的特征性表现,可作为CVM诊断的常规检查序列。 Objective To explore the clinical value of T2 star weighted angiography (SWAN) sequence in the diagnosis of cerebral venous malformation (CVM). Methods The MRI data of 15 patients with CVM were retrospectively analyzed. The patients were all examined by conventional scan, SWAN and enhanced scan. Results Among 15 cases of CVMs, 11 cases were located in supra tentorium, 4 cases were located in infra tentorium. MRI showed the draining veins demonstrated hypointensity on T1WI and hyperintensity on T2WI in 12 cases and in the other 3 cases, the draining veins demonstrated hypointensity on T1WI and hypointensity on T2WI;and all medullary veins demonstrated hypointensity on T1WI and hypointensity on T2WI. Both draining veins and medullary veins were clearly shown as hyperintensity on SWAN with the typical “Caput Medusas” characteristic. Multiple medullary veins converged into drain veins were shown in enhanced scan. Conclusion Comparing with conventional and enhanced MRI, SWAN could display the features of CVM more clearly and sensitively, which indicates that SWAN could be used as a routine sequence in diagnosing CVM.
