移植后早期PET-CT在霍奇金淋巴瘤预后评估中的作用

摘要 目的探讨自体造血干细胞移植(ASCT)后早期正电子发射计算机断层显像-电子计算机X射线断层扫描技术(PET-CT)检查对难治、复发的霍奇金淋巴瘤(HL)患者预后的意义。方法选取本院2007年11月至2013年3月收治的12例行ASCT的HL患者作为研究对象,回顾性分析其行ASCT后1个月PET-CT检查中最大标准摄取值(SUV值)与预后的相关性。结果挽救性化疗后完全缓解6例,部分缓解4例,难治2例。移植后完全缓解7例,部分缓解3例,难治2例。到最后随访时,9例(75.0%)患者生存,3例(16.3%)患者死于疾病进展。行ASCT后1个月PET-CT检查阳性3例。移植后PET-CT扫描阴性组总体生存率[83.3%(10/12)]及无进展生存率[75.0%(9/12)]明显优于阳性组[33.3%(4/12)、0],差异均有统计学意义(P<0.05)。结论 PET-CT可以作为ASCT的预后指标。 Objective To investigate the prognostic role of early positrone mission tomography-computed tomography (PET-CT) examination for the refractory and recurrent Hodgkin's lymphoma (HL) after autologous stem cell transplantation (ASCT). Methods Retrospective analysis was conducted on correlation between maximum SUV and prognoss by PET-CT 1 month after ASCT of 12 HL patients ,who were treated by ASCT from November 2007 to March 2013 in the hospital. Results After retrospective chemotherapy, 6 cases were with complete remission, 4 cases with partial remission and the other 2 cases were refractory; after transplantation, 7 cases were with complete remission, 3 cases with partial remission and the other 2 cases were refractory; at the end of follow-up ,9 cases were survival (75.0%), and the other 3 cases died due to progression of disease (16.3%). DG-PET positivity was found in 3 patients 1 month after ASCT. After transplantation, the overall survival [83.3% ( 10/12)] and progression free survival[75.0%(9/12)] of the negative group of PET-CT were obviously higher than those of the positive group,and the difference had statistically significant (P〈0.05). Conclusion PET-CT may be deemed as one of prognostic indicators of ASCT.
