摘要 背景 虽然恢复冠状动脉血流是挽救缺血心肌的重要措施,但是再灌注本身可以导致再灌注心律失常的发生,它与细胞内钙超载密切相关,钙/钙调蛋白依赖的钙调蛋白激酶Ⅱ(calcium/calmodulin-dependent protein kinaseⅡ,CaMKⅡ)是一种重要的丝/苏氨酸蛋白激酶,活化的CaMKⅡ通过多种方式调节胞内Ca2+的浓度. 目的 探讨CaMKⅡ的结构、功能及CaMKⅡ磷酸化目的蛋白对再灌注心律失常的影响. 内容 CaMKⅡ可能是再灌注早期发生心律失常的重要信号分子,CaMKⅡ功能异常导致钙稳态失衡,是再灌注心律失常的重要原因. 趋向 在缺血/再灌注的过程中选择合适的药物减轻钙超载,进而调节CaMKⅡ磷酸化目的蛋白的过程可能是未来治疗再灌注心律失常的重要靶点. Background Although coronary reperfusion in ischaemic myocardium is the only prerequisite of heart salvage,but reperfusion may lead to reperfusion arrhythmias.Cytosolic Ca2+ overload is a significant mechanism for the severe life-threatening arrhythmias.Ca/calmodulin-dependent protein kinase Ⅱ (CaMK Ⅱ)is a serine/threonine kinase,which can adjust the intracellular calcium concentration ([Ca2+]i) via various pathways.Objective To explore the structure and function of CaMK Ⅱ and the impact of CaMK Ⅱ-dependent phosphorylation of the target protein on reperfusion arrhythmias.Content CaMK Ⅱ is a main signal molecule for early reperfusion arrhythmias.CaMK Ⅱ dysfunction can lead to deregulation of intracellular calcium homeostasis,finally cause reperfusion arrhythmias.Trend The choice of appropriate drug on the CaMK Ⅱ and its relevant target protein may be important therapeutic approach for reperfusion arrhythmias.
