卵巢交界性上皮性肿瘤的预后分析

Analysis of epithelial borderline ovarian tumors prognosis

目的:探讨卵巢交界性上皮性肿瘤影响预后的因素。方法:回顾分析71例卵巢交界性上皮性肿瘤患者的临床资料,采用单因素和多因素分析方法分析影响复发和预后的因素。结果:BOT患者的5年总生存率97.0%,5年无瘤生存率为94.0%。单因素分析发现,FIGO分期、微浸润、腹膜种植、手术切除类型(肿瘤剥除与附件切除)、肿瘤包膜破裂、双侧卵巢受累与卵巢交界性肿瘤的预后有关(P<0.05)。COX多因素模型分析提示,手术切除类型、肿瘤包膜破裂是影响预后的独立因素(P<0.05)。结论:BOT发病年龄轻,预后良好,其复发与肿瘤FIGO分期、微浸润、腹膜种植、手术切除类型、包膜破裂等相关。对有高危因素者术后需长期密切随访。

Objective：To evaluate prognostic factors of epithelial borderline ovarian tumors. Methods：The clinical informations of 71 patients were retrospectively analysed. The clinico-pathological characteristics upon recurrence were evaluated by univariate and multivari- ate analyses. Results： 5-year overall survival （OS） and disease free survival rates （DFS） were 97.0% and 94.0%, respectively. Univariate analysis showed that the FIGO stage, microinva- sion, peritoneal implants, excisional type （ eystectomy or adnexectomy）, rupture and bilaterality were signficantly related to recurrence （P〈0.05）. COX regression analysis indicated that the excisi0nal type and rupture were independently prognostic factors （ P〈0.05 ）. Conclusions： BOT frequently affect younger patients and has an excellent prognosis. The FIGO stage, microinva- sion, peritoneal implants, excisional type and rupture were important factors of recurrence and prognosis. Thorough follow-up is warranted, especially for the patients who have high risks of re- currence
