抗α-胞衬蛋白IgA和IgG抗体联合检测诊断干燥综合征的价值

Value of joint detection of anti-α-fodrin antibody IgA and IgG to the diagnosis of Sjgren's syndrome

目的探讨抗α-胞衬蛋白IgA和IgG抗体联合检测在干燥综合征(Sjgrens syndrome,SS)诊断中的价值。方法32例原发性干燥综合征(primary SS,pSS)患者(pSS组),51例继发性干燥综合征(secondary SS,sSS)患者(sSS组),39例系统性红斑狼疮(systemic lupus erythematosus,SLE)患者(SLE组),51例类风湿关节炎(rheumatoid arthritis,RA)患者(RA组)和60例体检健康者(对照组),各组采用ELISA法定量检测血清抗α-胞衬蛋白IgA、IgG抗体的表达情况。结果 pSS组、sSS组、SLE组、RA组和对照组血清抗α-胞衬蛋白IgA抗体阳性表达率分别是90.6%、72.5%、12.8%、19.6%、5.0%,血清抗α-胞衬蛋白IgG抗体阳性表达率分别是50.0%、76.4%、30.8%、38.5%、5.0%,除SLE组、RA组外,余各组血清抗α-胞衬蛋白IgA和IgG阳性表达率比较差异均有统计学意义(P<0.05);pSS组抗α-胞衬蛋白IgA抗体阳性表达率高于IgG抗体(P<0.05);SLE组、RA组抗α-胞衬蛋白IgA抗体阳性表达率低于IgG(P<0.05)。结论抗α-胞衬蛋白IgA和IgG抗体有助于干燥综合征的诊断和鉴别诊断。

Objective To investigate the value of joint detection of anti-α-fodrin antibody IgA and IgG to the diagnosis of Sjogren＇s syndrome（SS）.Methods The expressions of serum anti-α-fodrin IgA and IgG were detected by ELISA in 32 patients with primary SS（pSS group）,51 secondary SS（sSS group）,39 patients with systemic lupus erythematosus（SLE group）,51 patients with rheumatoid arthritis（RA group）and 60 health volunteers（control group）.Results The positive rates of serum anti-α-fodrin IgA were 90.6%,72.5%,12.8%,19.6% and 5.0%,and the positive rates of serum anti-α-fodrin IgG were 50.0%,76.4%,30.8%,38.5%and 5.0%in pSS,sSS,SLE,RA and control groups,showing significant differences among groups（P〈0.05）except between SLE and RA groups（P〈0.05）.The positive rate of serum anti-α-Fodrin IgA was higher than that of IgG in pSS group（P〈0.05）,and lower than that of IgG in SLE and RA groups（P〈0.05）.Conclusion Anti-α-fodrin IgA and IgG help the diagnosis and differentiate diagnosis of SS.
