埃索美拉唑持续泵入治疗急性上消化道大出血疗效观察

Clinical observation on esomeprazole in the treatment of acute upper gastrointestinal bleeding with continuous pumping administration

目的观察埃索美拉唑持续泵入治疗消化性溃疡合并急性上消化道大出血的临床疗效。方法对我院2012年1月-2014年3月收治的45例消化性溃疡合并上消化道大出血患者,随机分为治疗组（n=23）和对照组（n=22）。两组均在扩容、口服去甲肾上腺素冰盐水、心电监测、吸氧、留置胃管等常规治疗同时,观察组首次给予埃索美拉唑80mg静滴,然后8mg/h持续泵入,连用72h;对照组首次给予奥美拉唑80mg静滴,然后8mg/h持续泵入,连用72h。结果观察组有效率为91.3%,对照组有效率为54.5%,两组比较差异有统计学意义有（x2=7.77,P〈0.01）。副反应方面两组均未发现明显副作用。结论埃索美拉唑持续泵入治疗消化性溃疡合并上消化道大出血疗效肯定,安全可靠,值得临床推广应用。

Objective To observe the clinical benefit rate of continuous pumping treatment of Esomeprazole on peptic ulcer with acute upper gastrointestinal bleeding. Methods In our hospital from January 2012 to March 2014, 45 patients of peptic ulcer with upper gastrointestinal hemorrhage patients were randomLy divided into observation group（n=23） and the control group（n=22）. Both groups were given routine treatments as the expansion, oral norepinephrine ice brine, ecg monitoring, oxygen, indwelling gastric tube and other conventional treatment. At the same time, the observation group were firstly given a dose of 0.3 mg esomeprazole with static drop, and then a dose of 4 mg/h with 72 h continuous pumping administration; Control group were firstly given a dose of 40 mg omeprazole with static drop, and then 4mg/h with 72 h continuous pumping. Results The treatment effective rate for observation group was 91.3%, while control group 54.5%, so it was characterized when compared two groups statistically difference（x2=7.77, P〈0.01）. In addition, there were not found obvious side effects in two groups. Conclusion The continuous pumping treatment of Esomeprazol has satisfactory effect on peptic ulcer with upper gastrointestinal bleeding, safe and reliable, worthy of clinical popularization and benefits to the peptic ulcer merge bleeding patients.
