老年高血压合并脑梗死中医常见证候分布规律的研究

目的分析老年高血压病合并脑梗死(恢复期)中医的常见症状体征及辩证分型,从发病机制方面入手研究,为中医预防治疗老年性脑梗死提供新的思路。方法收集老年性高血压合并脑梗死病患的临床病例、Logistic回归分析、x2检验等统计方法,分析中医方面症状体征,运用中医理论初步探讨高血压病合并脑梗死在老年患者中的发病机制及其证候规律。结果肢体麻木、眩晕、神疲、五心烦热、、口干、懒动懒言、盗汗、失眠、便秘比较,差异有统计学意义(P<0.05)。其中以五心烦热为突出表现多次出现在高血压病脑梗死组中,相关程度高;风痰火亢证、风痰瘀阻证、阴虚风动证所占比例较大;而舌脉以暗红舌、红舌、黄腻苔,弦滑脉为主。结论老年性高血压合并脑梗死(恢复期)的病因病机以"血脉损伤"为切入点,使中西医在有关脑梗死的病因认识上,"血脉损伤,脑脉痹塞"的理论达到共识,为脑梗死的防治提出新思路。

Objective To analysis of senile hypertension with cerebral infarction（recovery） of common symptoms and signs of traditional Chinese medicine and dialectical points,from aspects of pathogenesis research,for the prevention and treatment of senile cerebral infarction new idea of traditional Chinese medicine. Methods Collected in elderly patients with hypertension with cerebral infarction clinical cases,logistic regression analysis,chi-square statistics method,analysis in terms of symptoms and signs of traditional Chinese medicine,on the basis of the theory of traditional Chinese medicine preliminary discussion of hypertension with cerebral infarction in elderly people in the pathogenesis and syndrome patterns. Results Numbness,dizziness,weakness,five upset hot,god,dry mouth,lazy lazy words,night sweats,insomnia,constipation（P〈0.05）,there was a significant statistical significance.With five upset hot for outstanding performance many times appeared in the hypertension cerebral infarction group,the related degree is high;Wind phlegm fire in port,the resistance to wind phlegm and blood stasis syndrome,Yin deficiency pneumatic proportion is larger;Pulse and tongue to dark red tongue,red tongue,yellow greasy moss,string is give priority to. Conclusion From the perspectives of blood relation ＂damage＂ to investigate senile hypertension with cerebral infarction（recovery） of etiology and pathogenesis,make on the cause of cerebral infarction in Chinese and western medicine, ＂blood damage,cerebral vein bi＂ theory to achieve consensus.For cerebral infarction prevention and control of new ideas are put forward.
