胃后壁间质瘤误诊为黄体破裂1例

Gastric stromal tumor misdiagnosed as rupture of the corpus luteum: A case report

胃间质瘤在临床较为少见,突向腔外生长的报道更是少见.现报道胃后壁突向腔外生长的间质瘤破裂出血并误诊为黄体破裂1例.患者,女,24岁,因中上腹痛12 h,加重2 h入院,治疗12 h后,症状无好转,并发心慌,血压下降,诊断性腹穿抽出不凝血,妇科诊断为:黄体破裂?急诊手术中子宫及附件未见异常,发现胃体底交界处包块,突向外侧生长,术后病理:胃间质瘤(中度危险).免疫组织化学:CD117(+),Dog-1(+).术后8 d治愈出院.

Gastric stromal tumor is a rare clinical entity, and that protruding outside the cavity is even more rare. Here we report one case of gastric stromal tumor located on the posterior wall misdiagnosed as rupture of the corpus luteum in a 24-year-old female patient whose chief complaints were ab- dominal pain for 12 h and pain aggravation for 2 h. After 12 h of treatment, the symptoms did not remit, and she developed palpitation and a blood pressure drop. The blood suctioned was incoagu- lable during diagnostic abdominocentesis. An initial gynecological diagnosis was rupture of the corpus luteum. However, no lesions were found in the uterus and the accessories during emergen- cy operation, but a lump on the posterior wall of the stomach, protruding outside the cavity, was noted. Postoperative pathology suggested gastricstromal tumor （moderate risk）. Immunohisto- chemistry revealed CDl17 （＋） and Dog-1 （＋）. The patient was discharged from the hospital 8 d after operation.
